Travel RN Outpatient Case Manager

Pay based on your experience, the client will offer you within the rates below(Pay every Friday)

  • $40-50/hr(1-3 years of outpatient clinic experience)
  • $50-55/hr(3-7 years of outpatient clinic experience)
  • $55-60/hr(7+ years of outpatient clinic experience)


Number of Positions: 1

  • Remote Alaska, 1.5 hour plane ride from Seattle

Start Date: 07/01/2025

Travel: Flights, Rental Car, and Lodging covered(no stipend provided)(the facility will book all of this for you)

Length: 6 month need

Schedule: Monday-Friday 8:00 am-5:00 pm



Requirements:

The Case Manager, clinic team and Director of nursing will identify:

  • The following Patient Populations known to be at high risk due or in need of close monitoring:
  • Elders
  • Hypertension (HTN) patients
  • Diabetes (DM) patients
  • Cancer Related Chemotherapy
  • HIV/AIDS
  • Asthma
  • Hepatitis
  • Chronically ill and at-risk pediatric patients
  • Others as deemed appropriate.
  • Patients who could benefit from case management assistance, such as: Team Conferences, Care Planning, Patient Contracting, and Family Meetings.
  • Participates in Case Management related quality improvement activities upholding AAAHC standards.
  • Seeking candidates with at least 2 years of experience in case management.
  • Familiar with Medicaid,medicare, changes/applications/procedure; referral management; resource management and familiar with the following programs: Cover My Meds, Comagine, Avality.


MINIMUM QUALIFICATIONS

  • Possess current valid State of Alaska driver's license with a record acceptable to KIC’s Insurance carrier.
  • Graduate Degree in nursing, from an accredited school of Nursing.
  • Recent outpatient experience - 3 to 5 years preferred


PREFERRED QUALIFICATIONS

  • Bachelor’s degree from an accredited school of Nursing.
  • Blend of home health, inpatient case manager, and ambulatory or primary care case manager


CERTIFICATION REQUIREMENTS

  • BLS
  • Able to obtain ACLS
  • Able to obtain PALS


LICENSE

  • Registered Nurse in the State of Alaska or eligible to practice in the United States as a commissioned officer of the U. S. Public Health Service.
  • Any State License


ABOUT THE FACILITY

We provide acute and chronic medical, dental, pharmacy, optometry, and behavioral health services—offering specialized women’s health and pediatrics care. In addition, we offer essential preventive services such as cancer screening, TB screening, diabetes screening, prenatal care, and health education. Since assuming the health care services, the Clinic has seen many positive changes, including patient visit increase, construction of a new freestanding, ambulatory health facility (April 2000), a new behavioral health facility (July 2018) increased funding through grant resources and expanded services, including a diabetes program, optometry, and lab services.
