Additional Details about the position

• Promote patients' independence by continuing the plan of care established by providers and teaching patients and families to understand conditions, medications, and self-care skills
• Network with providers and agencies to provide ongoing follow-up and care services,
• Coordinate patient appointments for outpatient services, specialty clinics, and external clinics, and confirm travel arrangements
• Obtain medical records from other facilities and providers and verify records are complete before forwarding to Health Information Management
• Process patient referrals from NSHC and Specialty providers and track through Cerner Referral Management
• Place orders per provider request for labs, radiology, travel, and referrals
• Plan and organize specialty clinics and other events in support of healthcare promotion
• Scope of work: cross coverage for triage nurse, DM RN, wound care, cancer nurse navigator. Broadly, able to perform all duties within the scope of practice for a registered nurse.

EXPERIENCE REQUIREMENTS

• RN with 3+ years of outpatient experience in family medicine 
• IHS Experience Plus
• Remote Medicine Plus
• Flexible Schedule Plus
• IHS, FQHC Outpatient experience

CERTIFICATION REQUIREMENTS 

• BLS (AHA) 
• Graduate of an accredited school of nursing

LICENSURE REQUIREMENT 

•  WA License
