Additional Details about the position
- Provide information on available resources, including Medicare, Medicaid, insurance, and community services, and make appropriate referrals
- Conduct psychosocial assessments and develop action plans in collaboration with patients, families, and interdisciplinary care teams
- Advocate for patients by coordinating communication among providers, insurers, and family members
- Support care coordination and logistics for services such as medical appointments, transportation, housing, and meals
- Assist with discharge planning and coordination of follow-up support services
- Order and coordinate Durable Medical Equipment (DME)
- Facilitate and participate in Swing Bed meetings and related care coordination activities
- Obtain and manage CT prior authorizations and retro prior authorizations
- Assist with billing-related processes, including documentation, ICD-10 coding support, and progress notes
- Identify, coordinate, and secure assisted living and long-term care placements
- Make Adult Protective Services (APS) reports in accordance with regulatory and clinical requirements
- Assist patients with Medicare enrollment and financial assistance applications
- Perform work primarily by phone while maintaining accessibility to inpatients and outpatients across hospital and clinic settings
Administrative
- Lead and oversee CQI initiatives to improve care quality.
- Monitor patient satisfaction and lead efforts to enhance patient/family experience.
- Prepare reports on improvement outcomes and continuity of care.
- Produce monthly department reports for leadership.
- Coordinate Section 504 services and accommodations for patients with disabilities.
EXPERIENCE REQUIREMENTS
- Experience supporting patients and families with individualized care in interdisciplinary healthcare settings.
- At least 2 years of supervisory experience, including staff oversight and budget/resource management.
- Ability to provide age-appropriate care across assigned units.
- Strong knowledge of human development across the lifespan (pediatric, adult, geriatric).
- Skilled in assessing and interpreting patient data to identify age-specific needs.
- Proven ability to deliver care aligned with organizational policies, procedures, and standards.
CERTIFICATION REQUIREMENTS
- Master of Social Work (MSW) from a school accredited by the council on Social Work Education (CSWE).
LICENSURE REQUIREMENT
- Licensed Master Social Worker (LMSW) required
- AK license required

This facility includes 21 village clinics that are accredited by Joint Commission and are staffed by Community Health aides. Our Community Health Aide Program (CHAP) plays a very important role in village health services. Our health aides can provide Preventative, Acute, Chronic, and Emergency care within their scope of practice using the Community Health Aide Manual (CHAM) and reporting to a referral Physician at the Kanakanak hospital. They work closely with our case management and assist many other departments that include, but not limited to: Optometry, Audiology, Diabetes, Medical Staff, Dental, and Infant learning to ensure our patients are getting the appropriate and essential care that they need.
