REQUIREMENTS

• Bachelor of Science degree and/or Doctor of Pharmacy (PharmD) from an accredited school of Pharmacy
• Five years professional experience with two years of management experience
• 1-year residency
• Three (2) years of pharmacist experience
• Minimum two (2) years in hospital practice - Preferred

CERTIFICATION REQUIREMENTS

• BLS

LICENSURE REQUIREMENT

• Current and valid state Pharmacy license, required
• AZ

We are a Tribal 638 site providing healthcare to the San Carlos Apache people. Our hospital is young and thriving.
