Additional Details about the position

• Develops care plans appropriate to the patients/families health care needs in conjunction with patient, family, physician, clinical operations team and third party payers including obtaining medical orders and approval of medical services.
• Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established time frames.

EXPERIENCE REQUIREMENTS

• Certified Home Health Care Experience Required
• EPIC or Netsmart Required
• One year of experience as a clinical pediatric nurse strongly preferred

CERTIFICATION REQUIREMENTS

• BLS

LICENSURE REQUIREMENT

• RN New York
