Unit Overview:
- 2 North: 25-bed Medical/Surgical unit
- Observation Unit: 7–14 beds (based on census)
- Med/Tele nurses support both 2 North and the Observation Unit. Nurses are may work on either unit based on staffing and patient needs.
- Patient Population: Adult to geriatric
Observation Unit (lower acuity, mostly independent with ADLs):
- Acute on chronic CHF
- Hypertension
- Syncope
- Electrolyte imbalances (hypomagnesemia, hypokalemia)
- Dehydration
- Post-op extended observation
2 North (varied acuity, independent to full assist):
- Hypertension
- Respiratory failure, pneumonia, COPD exacerbation
- Controlled A-fib
- Hyponatremia
- COVID, sepsis
- Hematuria with CBI
- Anemia
- Generalized weakness
- End-of-life care
- Total joint replacements & minor surgical procedures
Telemetry:
- Basic telemetry monitoring
- Centralized 24/7 monitoring by telemetry techs
Drips/Infusions:
- Heparin, magnesium, potassium
- Continuous IV fluids
- No advanced titratable cardiac drips
Respiratory Support:
- No mechanical ventilation
- Higher-acuity respiratory cases managed off unit
Staffing & Ratios:
- Observation: 1:5–6 (days/nights)
- 2 North: 1:5 (may flex to 1:6)
- Support Aides: 3 (2 North), 1 (Observation)
- Unit Clerk: 0900–1700
- 24/7 Services: Respiratory therapy, phlebotomy, security
- No IV team (nurses start IVs)
Provider & Ancillary Support:
- Dedicated night-shift provider
- On-call surgical providers (rarely needed)
- Pharmacy remote overnight (Cardinal Pharmacy)
- Most meds in Pyxis; some nighttime reconstitution required
- Day-shift SWAT/wound-care nurse available
Equipment & Systems:
- Continuous telemetry
- Baxter IV pumps
- Pneumatic tube system
Leadership & Escalation:
- Unit leadership available weekdays + as needed
- 24/7 nursing supervision
- Rapid Response/Code Team assigned per shift
Orientation:
- Begins on day shift with educator
- Includes glucometer competency & workflow review
- Floor orientation with preceptor
- Night-shift travelers will complete the initial day with the educator and then transition onto PM shifts with a preceptor for the balance of orientation.
EXPERIENCE REQUIREMENTS
- Experience with Epic is strongly preferred but not required
- Must be skilled to handle the full scope of patient care without support
- Able to work independently and provide care across a variety of units. Nurses in these units should have a strong foundation in med/surg nursing, be skilled in telemetry monitoring, and be adept at managing a diverse patient population.
