• Patients are a high-morbidity tribal population, dealing mainly with coordination of care and with hospital discharges, access to resources for patients/staff, this role is mostly community-based, working in conjunction with community health

EXPERIENCE REQUIREMENTS

• 10 years of RN experience required minimum

• Any State License
