Pharmacist in Ambulatory Care/Outpatient: 8 hour shifts w/30 minute lunch
- 8:00 am - 4:30 pm OR 9:00 am - 5:30 pm
Pharmacist in Acute Care/ Inpatient:10-hour shifts 4 days per week with 3 days per week scheduled off. Hours needed to cover emergency leave, vacancies, and other staff shortages
- 7:30 am - 6:00 pm
- 10-hour shifts with 30-minute lunch
- may include weekends and holidays if needed
Additional Details about the position
- Review patient charts to ensure medication orders align with clinical notes, lab results, vitals, and allergy information.
- Process, fill, and verify prescription orders for accuracy and completeness.
- Provide patient counseling and education on medication therapy and proper use.
- Collaborate with pharmacy staff and healthcare providers to ensure safe, timely, and effective pharmaceutical care.
- Respond to provider inquiries and make appropriate clinical interventions regarding drug selection, dosing, and administration.
- Identify and manage adverse drug reactions, medication errors, and emergency situations, including initiating appropriate interventions.
- Handle phone communications, refill requests, and input external prescriptions into the system.
- Perform controlled substance audits and assist with inventory management, including stocking medications.
- Prepare and manage IV admixtures and support inpatient drug distribution in accordance with policies.
- Administer immunizations (with required certification).
- Participate in required training, safety programs, and ongoing competency assessments.
EXPERIENCE REQUIREMENTS
- One (1) to three (3) years of clinical pharmacy experience in ambulatory or inpatient care.
- Bachelor or Doctor of Pharmacy Degree from an Accredited Pharmacy School Program.
- Completion of a PGY-1 residency or equivalent clinical experience preferred
CERTIFICATION REQUIREMENTS
- BLS
- Certification in immunization delivery or other clinical pharmacy services (e.g., diabetes, asthma)
LICENSURE REQUIREMENT
- Applicants must have an active, unrestricted, and free of disciplinary action license as a Pharmacist in a State, District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States.
