Schedule: 40 hours per week

• Monday - Friday
• 8a - 4:30p 
• 30 minute lunch is required
• 38 hours - flex to 40 guaranteed 
• Holiday Rotation: none
• Will rotate 1 weekend every 6-8 weekends 
• Meets productivity expectations of an average of 4.5 visits per day

Additional Details about the position

• Charting/Equipment: Laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication.
• Unit Specific Skillset/Unit Overview: Evaluates, treats and plans patient care in accordance with Physical Therapy Standards of practice. 
• Population served includes adult and geriatric patients. Documents to meet regulatory and reimbursement requirements
• Hard worker, Resilient and Manages Change well, Social candidates.
• Scrub Colors: any or Business casual

EXPERIENCE REQUIREMENTS

• Minimum Experience: Home Health experience, training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis. Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy. 

CERTIFICATION REQUIREMENTS 

• AHA or ARC BLS
• NPI required
• Bachelor's Degree in Physical Therapy

• WI
