Pay Rate: $2,200.00 - $2,200.00 gross per week

Schedule: Monday-Friday 8am-5pm

• On site for just the 1st week or 2 max, then fully tele-health

Additional Details:

• 13 patients a month very light work
• Identify people and communities in need of help.
• Conduct comprehensive assessments to determine clients clinical diagnosis and level of care needed.
• Create and implement treatment plans using evidence-based practices and interventions that are culturally and ethically appropriate.
• Empower clients to cope with changes and challenges in their lives that create barriers to wellness.
• Collaborate with the Behavioral Health Case Manager to research, refer, and advocate for community resources, such as food stamps, childcare, and healthcare to assist and improve a clients well-being.
• Provide crisis intervention as needed to respond to mental health emergencies.
• Ensure clients situations have improved by providing follow-up care.
• Maintain case files and records in a confidential and secure manner.
• Develop and evaluate programs and services to ensure that basic client needs are met.
• Comply with all local, state, and federal mandates.

REQUIREMENTS/ CERTIFICATION REQUIREMENTS

• Active LCSW in the state of Alaska
• Education: Masters Degree in Social Work
• Two years of increasingly responsible experience, or any equivalent combination of related education and experience.
• Specialized Skills: Ability to make psychosocial assessments; develop and implement viable treatment plans, trauma treatment experience.
• FQHC Experience

ABOUT THE FACILITY

This units cover 87,828 square miles. The service area consists of a hub city of 55 surrounding remote villages which are sparsely populated areas. These areas have a population of 25,878 per the 2014 United State Census Bureau. This clinic received its 2014 Level III certification as a Patient-Centered Medical Home in 2016.
