• RN Case Manager for Elders In Home Support Services Department- This is not a regular clinic job

Start Date: 01/05/2026

Length: 13 Weeks

Schedule: Days / 8:00 AM - 5:00 PM , 40 Hours a Week

• Working with 8-10 clients in the field

Call: Every 4th weekend from Friday night at 5pm to Monday morning at 7pm

RPMS- EHR

Services Offered:

• Professional Nursing Case Management Oversight
• Medical Social Services – assistance with applications, hospital discharges, case planning, Medicare,
• Medicaid, disability questions, or other related services
• Life Alert – Medical Monitoring Emergency System
• Elder and Vulnerable Adult tailored Care Plans
• Assistance with Activities of Daily Living (ADL)

ADLs: Ambulation/Positioning after transfers, Dressing, Grooming, Bathing, Personal Hygiene, Toileting, Meal Preparation, Essential Shopping, Light housework, Laundry, Medication Reminders, Transport to and from Medical Appointments, Wound Care, End of Life Support.

Additional Details/REQUIREMENTS:

• Needs Outpatient Experience
• The provider has their own cars and takes adults and in home elderly patients to appt and sometimes the caretaker takes them and does assessments and there is high possibility of new clients the provider will help with.
• Providers Have to be willing to see problem Opioid patients, potentially call APS to file incident reports if they are unable to handle situation and file incident reports. These types of patients are elderly, so provider will be entering their home and it's not for the faint of heart.
