Additional Details about the position

• The LPN is responsible for utilizing the principles of Relationship-Based Care while performing duties.
• The LPN is responsible for initial data collection on all residents.
• The LPN participates in implementing person-driven care plans as developed by RN.
• Wound care, IV Drips, Skilled Nursing
• Managed LTC Caseloads (Chronic illness, rehab, dementia)
• Care Transitions, CNA Supervision

EXPERIENCE REQUIREMENTS

• LTC experience preferred, or geriatric experience
• Clinical experience preferred 

LICENSURE REQUIREMENT 

• ND or Compact State License
